OLD FORGE — Registration for the 2016-2017 school year for kindergarten will be held in the high school auditorium, 300 Marion St., on Tuesday, March 15 and Thursday, March 17.

Registration hours are from 9 to 11 a.m. and 1 to 2:45 p.m. on both days.

In order to facilitate the registration process, parents are asked to have all required documentation at time of registration. Registrations will not be complete until all the documentation is provided to the district.

Children are eligible for kindergarten in the school district if they are five years of age on or before Sept. 30.

Items required for registration include:

Proof of age: Requirements for age proof include an original birth certificate, original Baptismal certificate or transcript of the record of Baptism – duly certified and showing the date of birth and written statement from a family physician on their letterhead attesting to the chronological age of the student.

Proof of district residency: Registrants must bring a current utility bill, the part with their name, address, and date on it. (i.e. PPL, Cable, Water, Oil, etc. and must produce at least two of the following (must be current and must have the current Old Forge address) — driver’s license, vehicle registration, vehicle insurance, bank statement, W2 form/Payroll stub or a health insurance statement.

If registrants rent a home or apartment they must bring a current utility bill, the part with their name, address, and date on it. (i.e. PPL, cable, water, oil, etc.) and must produce at least two of the following (must be current and must have the current Old Forge address) — driver’s license, vehicle registration, vehicle insurance, bank statement, W2 form/payroll stub or a health insurance statement.

In addition to the above a copy of the registrant’s current lease with their child/children’s name on it is also needed.

If the student/family lives with another district resident they must bring multiple occupant application completed and notarized at time of registration, the homeowner/lessee must provide a current utility bill, the part with their name, address, and date on it (i.e.: PPL, Cable, Water, Oil, etc.) and both the homeowner/lessee and the parent/guardian of the student must produce at least two of the following — driver’s license, vehicle registration, vehicle insurance, bank statement, W2 form/Payroll stub or health insurance statement.

Immunization records: Registrants must provide records for immunizations against the any of the following — Diphtheria, Pertussis, Tetanus, Polio, Measles, Rubella, Mumps, Hepatitis B, Varivax and/or a history of chicken pox, or Tuberculin test and/or tuberculin risk evaluation, if born outside of the United States.
