NANTICOKE — A nurse supervisor at a healthcare and rehabilitation center surrendered Friday on a charge she neglected to properly care for a patient who died.

Kelly Levandowski, 39, of West South Street, Nanticoke, allegedly failed to issue orders for one-on-one care or perform 15-minute checks after a 72-year-old woman pulled a tracheostomy tube from her neck while a patient at Guardian Healthcare and Rehabilitation Center on Old Newport Street on June 20, 2013. The elderly woman died the same day.

Levandowski, who was accompanied by her attorney John Pike, was arraigned by District Judge Donald Whittaker and released on $15,000 unsecured bail.

Levandowski was charged after an investigation by the Bureau of Criminal Investigations Care-Dependent Neglect Team with the state Office of Attorney General.
