OLD FORGE — Kindergarten registration for the 2017-18 school year will be held from 9 to 11 a.m. and 1 to 2:45 p.m. Monday, March 13 and Tuesday, March 14 in the Old Forge High School auditorium, 300 Marion St.

In order to facilitate the registration process, parents are to have all required documentation at time of registration. Registration will not be complete until all the documentation is provided to the district.

Forms can be found at Old Forge School District website and should be printed out, completed and brought to registration, along with required documentation.

The following required documentation is needed for registration:

• Proof of age (children are eligible if they are five years of age on or before Aug. 31). Acceptable forms include original birth certificate, original baptismal certificate or transcript of the record of baptism (duly certified and showing the date of birth), written statement from a family physician on their letterhead attesting to the chronological age of the student.

• Proof of district residency. Acceptable forms include current, dated utility bills with parent’s name and address.

• The new registrant (parent/guardian) must also produce at least two of the following (must be current and must have the current Old Forge address): driver’s license, vehicle registration, vehicle insurance, bank statement, W2 form/payroll stub and health insurance statement.

If the parent/guardian rents, bring a copy of current lease with child/children’s name on it, in addition to the above.

If the student/family lives with another district resident, a multiple occupant application must be completed and notarized at time of registration and both the homeowner/lessee and parent/guardian of the student must produce at least two of the documents mentioned above.

Student immunization records must also be submitted for diphtheria, pertussis, tetanus, polio, measles, rubella, mumps, Hepatitis B, VARIVAX (a history of chicken pox is also acceptable) and a tuberculin test (or tuberculin risk evaluation if born outside the United States).
